Trinity Health Of New England seeks aNurse Practitioner or Physician Assistant to join theCardiology Heart Failure team at Saint Francis Hospital and Medical Center.
RESPONSIBILITIES:
- Seeing patients admitted with heart failure in consultation with the cardiology team
- Performing advanced heart failure consultations
- Working with the interdisciplinary team made up of hospitalists, social workers, nurses, Cardiologists
- Working as an extension of the physician in managing patients with heart failure.
- Skillset includes: Management of diuretics, Guideline directed medical therapy initiation, Management of cardiogenic shock
- Attending interdisciplinary rounds daily
The successful candidate will:
- Be interested in heart failure care
- Have Cardiology/Heart Failure experience (preferred but not required)
- Hold an Advanced Practice Nurse or Physician Assistant CT state license.
