• Certifications: Board Certified in Anesthesiology; Texas State License; ACLS, BLS

• Credentialing time frame: Up to 120 days from completion of facility application
• Schedule: 5x8 M-F, full-time preferred. No call. No weekends. Will consider 10s, though 8s are preferred
• Supervised Model
• Length of Assignment: 6-12 months
• Case Types: Ortho, urology, general surgery, GI, GYN, ENT, and cath lab (A-fibs, TEEs, ICD implants)
• Cross-Credentialing: Additional network facilities for added case variety
• Start: As soon as credentialed
