Position Overview

Ambulatory Clinic RN Care Coordinator - Cancer Center. The RN Care Coordinator is a licensed registered professional nurse who provides ethical, evidence‑based, safe and compassionate nursing care. The role focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs, performing clinic‑related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator supports physicians by communicating with patients/families/caregivers and relaying information to the physician, ensuring the patient/family understands the plan of care, verifying orders, and notifying the physician of results.

Responsibilities

• Management of patient flow, triage, and direct patient care.
• Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
• Managing patient outcomes and setting patient goals in collaboration with providers.
• Providing nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.
• Supporting patient self‑management of disease.
• Coordinating continuity of patient care following hospital admission, discharge, and ER visits.
• Managing high‑risk patient care, including patients with multiple comorbidities or high risk for readmission, and maintaining a registry.
• Developing relationships with patients and using assessment skills to identify physical, psychosocial, financial, and environmental health care needs.
• Educating patients and ancillary personnel about health care needs, safety issues, and benefits, including patient education.
• Utilizing electronic tools such as EMR to document assessments, clinical findings, treatment plans, and progress toward goals.
• Observing, recording, and reporting patient condition and reactions to drugs and treatments to physicians.
• Educating patients/families about diagnostic procedures, medications, nutrition, and health maintenance.
• Maintaining and reviewing patient records, charts, and other pertinent information.
• Recording test and examination results, posting them, and following up with patients.
• Responding to or referring incoming patient treatment–related phone calls.
• Instructing patients and families regarding medication and treatment instructions.
• Maintaining patient confidentiality.
• Documenting patient care using EMR in compliance with practice standards.
• Performing professional, clinical, and technical competencies of the assigned unit or department.

Qualifications

• High School Graduate or Bachelor Degree in Nursing.
• 2 or more years of RN experience.
• Previous medical office experience is preferred.

Licensure and Certification

• Licensed Registered Nurse (LRN) - Multi‑State – State Board of Nursing.
• Must have a current active multistate RN license in good standing in the state where the applicant resides.
• Staff hired prior to 2/1/2024 must obtain a multistate RN license from the state in which they live at the time of their next RN license renewal by 4/30/2026.
• Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) within 14 days.

Skills and Knowledge

• Basic typing and word processing ability.
• Excellent analytical, critical thinking, and problem‑solving skills.
• Ability to coordinate multi‑disciplinary consults and/or referrals.
• Excellent communication and coordination skills.
• Ability to work independently and as part of a team with a strong sense of accountability and a high level of professionalism.
