UVA Physicians Group is seeking an Ambulatory Care Clinical Pharmacist for two of our primary care practice locations: UVA Primary Care Riverside in Charlottesville, VA, and Medical Associates of Louisa in Louisa, VA.

The Ambulatory Care Clinical Pharmacist provides evidence‑based pharmaceutical care to patients in the ambulatory clinical setting. Their responsibilities include designing, implementing, assessing, monitoring, and documenting therapeutic plans using best evidence, while aiming to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams. Pharmacists may perform patient assessments, participate in collaborative practice agreements, provide medication management, order, interpret, and monitor medication therapy‑related tests, coordinate care, provide education, and document care plans in the medical record.

Minimum Requirements

  • Education: Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy (degree required).
  • Experience: PGY-1 residency required or equivalent clinical experience.
  • License/Certification: Commonwealth of Virginia State Board of Pharmacy License required; American Heart Association Health Care Provider BLS certification required; Board Certification or equivalent certification specific to clinical area highly encouraged.

Physical Demands

Job requires prolonged sitting; repetitive motion: typing. Proficient communicative, auditory and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull < 20lbs; may be exposed to fumes, chemicals, blood/body fluids, and infectious disease.
