Community Health Network is seeking an ambulatory anticoagulation clinical pharmacy specialist to join our care team. You will manage DOACs and warfarin within approved protocols, provide patient and staff education, and participate in guideline development and quality initiatives.
You will mentor students and residents, collaborate with physicians and RN teammates, and support anticoagulation clinics both in person and virtually to improve patient outcomes.
