Social Worker
This role involves providing acute inpatient medical case management, including discharge planning in a large hospital setting. The position requires a high level of expertise and quick adaptability to work efficiently.
Responsibilities:
- Manage patient caseload of 15-20 patients per day.
- Coordinate with respiratory therapy and pharmacy for discharge planning as needed.
- Participate in minimal orientation and integrate quickly with the case management team.
Requirements:
- ACTIVE LMSW or LCSW and MS State License.
- Minimum 2 years of acute inpatient medical case management experience with recent experience in a large hospital.
- Proficiency in EPIC charting and strong computer skills.
- Previous travel experience in a level 1 facility.
- BLS Certification.
- Flu vaccine required, only medical exemption with documented evidence accepted.
