Job Description:
The Hospice teams support patients and family members through the final stages of their illness. Services are offered in various settings, including the patient's residence, skilled nursing facilities, assisted living facilities, and adult foster homes. These include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, hospice volunteers, and bereavement support.
Requirements:
- Oregon LMSW or CSWA licensure required.
- Healthcare Provider Level BLS required.
- This position requires the use of the employee's personal automobile. A valid driver's license and auto liability/property damage insurance are required by law, and the vehicle must be maintained in good working order.
- Oregon LCSW preferred.
- Experience as a social worker in a hospital or medical setting preferred.
