Position Highlights:
Join an experienced team of 1 MD, 9 nurse case managers, 5 clinical pharmacists, and 1 nurse supervisor
Primary Location: Sentara Norfolk General Hospital
Schedule: Mon - Fri, 8:30AM - 5:00PM, no weekends or holidays and working from home is available
Duties: Perform health risk assessments, re-write prescriptions
Review 150 patients per month
Work with Sentara Medicaid only
1 year experience required as APP in Family Medicine
Experience in Rheumatology or Infectious Disease preferred
