To support a growing Medicare and Medicaid care management program, the full-time Remote RN Case Manager will conduct comprehensive assessments, develop individualized care plans, and coordinate care for members with chronic medical and behavioral health conditions. Key responsibilities Conduct comprehensive assessments of members' healthcare needs and develop individualized care plans Provide ongoing care coordination and connect members with healthcare resources and support services Document care management activities accurately in the electronic health record while meeting regulatory requirements Required qualifications Active and unrestricted RN license in Alabama or a Compact RN license in the state of residence Associate's degree in nursing (ASN) with a minimum of 3 years of nursing experience At least 2 years of case management, discharge planning, or home healthcare coordination experience Proficiency in Microsoft Office Suite Previous remote work experience
