Working remotely, the part-time Alabama Licensed RN Case Manager will assess patient health status, establish care goals, and collaborate with multidisciplinary teams to address medical, functional, and social barriers to care. Key responsibilities: Assess the health status of members and establish goals to meet their healthcare needs Plan, implement, and evaluate responses to the plan of care while collaborating with the multidisciplinary team Document patient interactions accurately in electronic medical records and provide education on health maintenance and disease prevention Required qualifications: Current unrestricted licensure as RN in Alabama 2+ years of experience as an RN Experience in assessing the medical needs of patients with complex behavioral, social, and/or functional needs Proven computer skills, including the use of Electronic Medical Records Access to reliable home internet and a home office to maintain HIPAA privacy
