Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess and coordinate patient and family needs during after-hours shifts, maintaining primary clinical oversight for assigned patients.
You will participate in IDG meetings, update plans of care with physicians and staff, and ensure accurate documentation while educating families on care options and symptom management.
