Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care during nights, weekends, and on-call hours. You will assess, treat, and support patients and families, coordinating with the Interdisciplinary Group (IDG) and physicians to ensure continuity of care.
You will lead after-hours visits, document thoroughly, and help educate families about disease progression and symptom management, while adhering to regulatory and quality standards.
