Afterhours Hospice Care Manager RN

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. You will assess and coordinate patient and family needs during nights, weekends, and on-call hours, while maintaining clinical oversight for assigned patients.

You will collaborate with physicians and the IDG, perform routine visits, admissions, and death visits, and ensure documentation and compliance across shifts. A compassionate, professional approach is required.
