Afterhours Hospice Care Manager RN

Heartland Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. This role ensures coverage, continuity of care, and clinical oversight for patients and families during nights, weekends, and on-call hours.

The RN assesses, coordinates patient and family needs, and collaborates with the IDG to align plans of care. You will educate families, document thoroughly, and ensure compliance with hospice standards.
