Afterhours Hospice Care Manager RN

Gentiva is seeking an Afterhours RN Care Manager to oversee hospice care outside standard hours. This role provides direct patient care during nights and weekends, coordinates with the IDG, and maintains primary clinical oversight for assigned patients.

The ideal candidate will have an active RN license, a strong understanding of hospice principles, and proficiency with HomeCare HomeBase. Travel and on-call duties may be required.
