Afterhours Hospice Care Manager RN

Heartland Hospice, part of Gentiva, is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. You will assess, coordinate, and oversee care for patients and families during nights, weekends, and on-call hours.

You will participate in the IDG, maintain care plans, and educate families while ensuring compliance, documentation, and quality outcomes across shifts.
