Afterhours Hospice Care Manager RN

Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will ensure coverage, continuity of care, and clinical management for patients and families during nights, weekends, and on-call hours.

The role emphasizes after-hours assessments, coordination with the IDG, and maintaining accurate documentation while supporting regulatory compliance and care quality.
