Gentiva Hospice is seeking an experienced Afterhours RN Care Manager to deliver high-quality hospice care outside standard business hours. You will assess, plan, and coordinate care for patients and families during nights, weekends, and on-call shifts, while collaborating with physicians and the IDG for seamless continuity of care.
You will maintain primary clinical oversight, provide live visits, ensure proper documentation, and support compliance with Medicare/Medicaid guidelines.
