Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours, including nights, weekends, and on-call shifts. You will assess needs, coordinate care, and maintain primary clinical oversight for assigned patients while collaborating with physicians and the Interdisciplinary Group (IDG).
You will conduct comprehensive assessments, educate families, manage care plans, ensure documentation, and support regulatory compliance.
