Northeast Arc is seeking a dedicated and compassionate Adult Family Care (AFC) Case Manager
Acts as an integral member of Adult Family Care’s interdisciplinary professional team providing case management services to AFC Members and Caregivers.
Major Responsibilities
- Conduct annual assessments and evaluations for existing members and caregivers
- Evaluate, continuously train and support AFC caregivers in conjunction with the AFC RN
- Maintain AFC placement caseload and participate in development, implementation, and ongoing review of AFC Plan of Care
- Document all communication and activity pertinent to AFC placement in member’s digital record within required timeframes
- Participate in planning and implementation of the member’s discharge and/or transition
- Participate in all required meetings per program standards, MassHealth regulations and additional meetings, trainings and conferences as requested by AFC management
- Perform other duties as assigned
Qualifications
- Bachelor’s degree and two years’ experience working with elderly or disabled persons
- Maintain CPR and First Aid certification and complete required programmatic trainings
- Bilingual in English and Spanish
- Must reside in the Merrimack valley / Lowell area
- Proficient computer skills with Microsoft Office and digital healthcare records
- Valid driver’s license and reliable transportation
Physical Demands
- Must satisfactorily complete a physical exam and tuberculosis screening within the first 12 months of employment, and thereafter receive a physical exam every 2 years
- Must drive to various locations for home visiting purposes
- Light physical effort might be required
