Advanced Practitioners - Primary Care
Job Details:

• Join a well-established adult medicine practice in a 100% outpatient primary care setting
• Open to Nurse Practitioners (FNP) and Physician Assistants (PA)
• Full-time, Monday through Friday schedule with flexible scheduling options available
• No call required
• Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans
• Develop plans for further evaluation of patient health needs, including referrals to specialists and other health team members
• Provide appropriate treatment for problems within the scope of practice and refer outside-scope issues to the supervising physician
• Work collaboratively with practice physicians and care team members under established guidelines
• Utilize state-of-the-art clinical and informatics technology including EPIC to promote improved patient outcomes
• Access to dedicated advanced practitioner support through the Office of Advanced Practice
• Mentorship and orientation programs available for newly hired providers and those new to practice
• Unrestricted State of Connecticut Medical License required (or eligible)
• Minimum 1+ year of experience in a primary care or relevant outpatient setting required
• APRN (Family Nurse Practitioner) or PA training required
