Advanced Practitioners - Primary Care
Job Details:

• Join a seasoned and collegial primary care team in the charming town of Ellington, CT
• Role is open to Nurse Practitioners (APRN) and Physician Assistants (PA)
• 100% outpatient primary care setting - Family Medicine
• Full-time, weekdays only with flexible scheduling options available including a 4-day work week option
• No call required
• Manage your own patient panel with protected admin time and robust support staff
• Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans
• Develop plans for further evaluation of patient health needs including referrals to specialists and other health team members
• Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
• Utilize EPIC and state-of-the-art clinical and informatics technology to promote improved patient outcomes
• Receive dedicated support and professional development from Primary Care Leaders
• Access to established mentorship and orientation programs for newly hired providers
• 1+ years of experience in a primary care or similar setting required
• Unrestricted State of Connecticut Medical License required (or eligible)
