n Atrium Health Wake Forest Baptist is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our inpatient Palliative Care team in Winston-Salem, North Carolina. This position offers the opportunity to work collaboratively with a multidisciplinary team focused on delivering high-quality, patient-centered care for individuals with serious and complex illnesses. The APP will provide inpatient consultation services while contributing to education, academic initiatives, and program development.n

n Key Responsibilitiesn

• n Provide comprehensive inpatient palliative care consultations and follow-up care.n
• n Collaborate with physicians, APPs, nursing staff, and interdisciplinary team members to develop and implement patient-centered treatment plans.n
• n Assist patients and families with symptom management, goals-of-care discussions, advance care planning, and end-of-life care needs.n
• n Participate in educational activities involving medical students, residents, fellows, and other healthcare professionals.n
• n Contribute to quality improvement, program development, and academic initiatives within the division.n
• n Maintain accurate and timely documentation in the electronic medical record.n

n Qualificationsn

n Requiredn

• n Graduate of an accredited Nurse Practitioner or Physician Assistant program.n
• n Current or eligible North Carolina licensure.n
• n Board certification as:n
  • n Acute Care Nurse Practitioner (ACNP), Primary Care Nurse Practitioner (NP), orn
  • n Physician Assistant (PA-C)n
• n Eligibility for hospital privileges at Wake Forest Baptist Medical Center.n

n Preferredn

• n Previous palliative care, hospice, critical care, or ICU experience.n
• n Completion of a Palliative Care Fellowship Program.n
• n Certification in hospice and palliative care, including:n
  • n Advanced Certified Hospice and Palliative Care Nurse (ACHPN), orn
  • n NCCPA Certificate of Added Qualifications (CAQ) in Hospice and Palliative Medicine.n

n Candidates without certification are expected to obtain ACHPN or CAQ certification within two years of employment.n
