Qualifications:
- Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Must have an active and unrestrictive TexasAPRN license
- Must have DEA license or eligible to obtain it within 2 months of date of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
- Proficient with EMR systems (eCW a plus!)
