OnPoint Medical Group seeks an Advanced Practice Practitioner to deliver coordinated, patient-centered care within our physician-led Colorado clinics. In this role, you will independently evaluate patients, order and interpret diagnostics, and manage acute and chronic conditions while collaborating closely with physicians, behavioral health, and ancillary teams. You will support long-term patient relationships, provide education and preventive care, and participate in quality and population health initiatives that improve outcomes across the Denver metro community.
Responsibilities
- Provide comprehensive primary and specialty care as an advanced practice provider in collaboration with physicians
- Conduct patient assessments, order and interpret diagnostic tests, and develop evidence-based treatment plans
- Manage chronic and acute conditions, including follow-up care and patient education
- Coordinate care with behavioral health, ancillary services, and community resources to support patient outcomes
- Document encounters and care plans accurately in the electronic medical record
- Participate in quality improvement initiatives and population health efforts across the practice
- Collaborate with multidisciplinary team members to support a patient-first, relationship-based model of care
- Precept or mentor clinical staff and contribute to clinical protocols and best practices
