Required Qualifications
Doctor of Pharmacy (PharmD) Or Bachelor's Degree in pharmacy.
5 Years experience in the advanced practice area; Or
3 Years completion of PGY1 residency and 2 years experience in advanced practice area; Or
2 Years completion of PGY1 residency and PGY2 residency.
Basic Life Support for Healthcare Professional (BLS ) - American Heart Association
California Pharmacist License - CA State Board of Pharmacy
Pharmacy-Based Immunization Delivery - Various-Employee provides certificate
Other Qualification Requirements Any BPS (Board of Pharmaceutical Specialties) Certification is required within 6 months of hire into this position. Advance Practice Pharmacist License-CA Board of Pharmacy is required within 12 months of hire into this position.
