Advanced Practice Nurse (Temporary)

Palliative Care Nurse Practitioner Assured Hospice, part of LHC Group and the Optum family of businesses, is hiring a Palliative Care Nurse Practitioner in Centralia, Washington .

You'll provide direct patient and family care through palliative care consultations for individuals with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.

Conduct comprehensive physical, emotional, psychosocial and spiritual assessments Develop individualized plans of care using evidence-based palliative medicine practices Gather and evaluate information from medical records, family history and caregivers Complete medication reconciliation, formulate diagnoses and initiate therapeutic interventions Evaluate patient outcomes and adjust care appropriately

RN/APRN licensure in Washington Nurse Practitioner/CNS with prescriptive authority Advanced practice palliative care certification (ACHPN)

1+ years as an APRN in Hospice, Palliative Care or Oncology
