Advanced Practice Clinicians - Nurse Practitioner or Physician Assistant
Job Details
- Provide comprehensive primary care services in a community health setting
- Work with underserved patient populations in a patient-centered medical home environment
- Manage your own patient panel and develop cultivating patient relationships
- Collaborate as part of a passionate care team dedicated to community health
- Work with certified bilingual Medical Assistant for translation and patient care support
- Utilize EPIC EMR system for patient documentation and care coordination
- Part-time position at 0.25 FTE (10 hours per week)
- Must hold Master's or Doctoral Degree as Nurse Practitioner or Master's Degree as Physician Assistant
- New graduates encouraged to apply with clinical rotations and community health experience
- Commitment to working with diverse patient populations regardless of citizenship status
