Advanced Practice Clinicians - Nurse Practitioner or Physician Assistant

Job Details

• Provide comprehensive primary care services in a community health setting
• Work with underserved patient populations in a patient-centered medical home environment
• Manage your own patient panel and develop cultivating patient relationships
• Collaborate as part of a passionate care team dedicated to community health
• Work with certified bilingual Medical Assistant for translation and patient care support
• Utilize EPIC EMR system for patient documentation and care coordination
• Part-time position at 0.25 FTE (10 hours per week)
• Must hold Master's or Doctoral Degree as Nurse Practitioner or Master's Degree as Physician Assistant
• New graduates encouraged to apply with clinical rotations and community health experience
• Commitment to working with diverse patient populations regardless of citizenship status
• HPSA Score for Primary Care: 18
