Job Title: Adult-Gerontology Nurse Practitioner (AGNP)

Location: Escondido, CA

Schedule: 8:00AM - 5:00PM x 5

Term: 13-week assignment with the potential for extension

Pay Rate : $110 - $115/hr

Astrya Global, a San Diego based medical staffing agency, is hiring a Adult-Gerontology Nurse Practitioner (AGNP) for a temporary assignment in Escondido, CA. These positions are 13-week contracts with the ability to extend.

Responsibilities for a Adult-Gerontology Nurse Practitioner (AGNP):

• Provide home-based hospice and palliative care as a Nurse Practitioner for patients with serious or life-limiting illnesses
• Conduct patient assessments, develop individualized care plans, and manage symptoms
• Perform face-to-face home visits, averaging 45–50 visits per month
• Conduct goals-of-care discussions and advance care planning, including POLST and advance directives
• Provide patient and family education regarding hospice and palliative care
• Serve as a resource and support for home aides and the interdisciplinary team
• Coordinate care with the interdisciplinary team and community physicians
• Participate in IDT meetings and quality improvement initiatives
• Collaborate closely with the Hospice & Palliative Care Medical Director
• Document all patient encounters using My Unity EMR
• Work Monday through Friday, with one Sunday per month (with a corresponding weekday off), 8:00 AM–5:00 PM
• Participate in rotating weekends, occasional on-call coverage, and 24/7 patient coverage as needed
• Maintain current California NP and RN licenses, DEA, NPI, MSN, AANP/ANCC certification, and BLS
• Maintain a minimum of 3+ years of hospice or palliative care experience
• Obtain ACHPN certification within one year of hire (preferred at hire)
• Maintain a valid California driver’s license and auto insurance for local travel to patient homes
• Spanish bilingual skills are a plus
