Advanced Crisis Case Manager | CPST Certified

Join a dedicated team providing intensive, goal-oriented support to individuals facing mental health crises and substance use challenges in the Midwest region. This role focuses on crisis stabilization, working closely with clinical and community partners to deliver high-intensity services aimed at helping consumers maintain community tenure.
Essential Responsibilities

• Deliver person-centered case management using proven methods to alleviate mental illness symptoms and enhance emotional and functional well-being.
• Provide crisis stabilization services both in the community and within Crisis Stabilization Units.
• Conduct trauma-informed risk assessments under the guidance of Qualified Mental Health Professionals to ensure safety.
• Apply de-escalation techniques effectively in high-stress situations.
• Collaborate regularly with community partners, caregivers, family members, physicians, and other providers until crisis resolution.
• Support clients in achieving treatment goals, coordinate with collateral contacts, provide medication education, and assist with daily living activities.
• Monitor clients one-on-one for safety as needed and participate in care coordination to arrange and oversee services.
• Develop plans to ensure clients attend mental health appointments and work towards successful reintegration into home, community, school, and outpatient services.
• Work schedules may include nights and weekends based on community crisis demands and coordination with management.
• Complete all required documentation accurately and promptly, adhering to privacy and security regulations regarding Protected Health Information (PHI).
• Maintain professional communication and relationships with clients, team members, contractors, physicians, peers, outside agencies, and the public.

Education and Experience

• Bachelor's degree preferred in Social Work, Psychology, or related fields.
• Associate's degree with two or more years of experience educating and teaching clients about self-care and improving daily living activities is acceptable.
• Total combined education and experience should equal four or more years.

Licenses and Certifications

• CPST Case Management training required.
• Completion of Adult Case Management training within 60 days of employment.
• Pass required background checks including KBI, DCF child abuse, adult abuse registry, and motor vehicle screenings.
• Valid driver's license, insurance, and reliable vehicle if driving is required.

Preferred Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
• Three to five years of professional experience in mental health services, crisis response, or related fields.
• Two or more years of experience educating clients on self-care and daily living improvements.

Skills and Abilities

• Familiarity with community mental health services.
• Strong organizational skills.
• Intermediate proficiency with Microsoft Office.

Physical Requirements
Medium work involving occasional exertion up to 50 lbs and frequent exertion up to 10 lbs. Expect to stand or walk 26-50% of the day, with or without accommodation.
