Nurse Practitioner
Carease Health - PA is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals in Butler County, PA. As a Nurse Practitioner , you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities, working collaboratively with our team to reduce hospitalizations and improve patient outcomes.
About the Role:
As a hybrid role, you will split your time between on-site visits with patients and remote work from home, supported by a dedicated scribe team for efficient documentation. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
Responsibilities:
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct on-site visits and remote monitoring to detect abnormalities and prevent hospitalizations
- Collaborate with our clinical team to develop and implement individualized care plans
- Ensure seamless integration with the facility's processes and protocols
- Demonstrate excellent communication, collaboration, and teamwork skills
Requirements:
- Active Nurse Practitioner license (in the state of practice)
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
