Responsibilities
- Provide comprehensiveprimary care services to adult patients.
- Evaluate, diagnose, andmanage acute and chronic medical conditions.
- Perform preventive care,health screenings, and routine assessments.
- Manage medications andmonitor treatment effectiveness.
- Develop individualized careplans and provide patient education.
- Support long-term diseasemanagement and promote healthy behaviors.
- Collaborate with physiciansand other members of the healthcare team.
- Maintain accurate and timelyclinical documentation.

Requirements
- Master’s or Doctoral degreefrom an accredited Nurse Practitioner program.
- Current, unrestrictedCalifornia Nurse Practitioner license.
- National Board Certificationas an Adult Nurse Practitioner or appropriate adult-focused primary carecertification.
- Current DEA registration oreligibility to obtain one.
- Experience providing primarycare in an ambulatory or community-based setting preferred.
- Strong clinical assessment,diagnostic, communication, and patient education skills.
