Emmanuel Hospice in Grand Rapids, MI is seeking an Admissions RN to join our faith-based, nonprofit team. In this role, you’ll complete timely hospice admissions, performing comprehensive assessments and clearly explaining hospice services to patients and families. You’ll collaborate with an interdisciplinary team to develop individualized care plans, coordinate smooth transitions from hospitals or facilities, and ensure accurate, compliant documentation.
