Deliver Patient Care:

- Prescribe medications and therapies as appropriate for each patient.
- Provide ongoing disease management, monitoring, and follow-up care.
Patient Education and Support:
- Empower patients and families through education about diagnoses, medications, and self-care practices.
- Provide preventive care including vaccinations, health screenings, and lifestyle counseling.
- Encourage proactive wellness strategies for improved long-term outcomes.
Collaborate with Healthcare Teams:
- Work closely with physicians, specialists, nurses, and ancillary staff to ensure coordinated care.
- Contribute insights during case reviews and care planning meetings.
- Offer clinical mentorship and support to fellow healthcare team members.
Monitor Patient Progress & Ensure Compliance:
- Regularly evaluate treatment effectiveness and adjust plans as needed.
- Maintain accurate documentation using EHR systems.
- Follow all HIPAA, state, and federal regulations to ensure patient safety and privacy.
Qualifications:
- Master's degree in Nursing from an accredited Nurse Practitioner program.
- Active NP license in the state of Kentucky.
- Board Certification as a Nurse Practitioner (FNP, AGNP, or equivalent).
- BLS and ACLS certifications preferred.
