Provide evaluation, treatment, and discharge planning for diverse adult and geriatric patients, including post-surgical, orthopedic, neurological, and cardiac cases. Collaborate closely with physicians, nurses, and rehab teams to create evidence-based, goal-oriented plans of care that promote mobility, functional independence, and safe transitions.
Support patient and family education, maintain accurate documentation, and uphold hospital quality, safety, and patient-centered care standards in a fast-paced acute care setting.
