Activities Director (Program and Outreach Director)
Lexington Country Place is seeking an Activities Director to join their team. The Program and Outreach Director reports directly to the Executive Director.

Purpose
The Program and Outreach Director is responsible for developing, leading, and overseeing a resident‑centered activities and outreach program that promotes and integrates the Phoenix Senior Living philosophy to incorporate wellness in all aspects of the seniors’ lives we serve. Ensuring all dimensions (emotional, sense of purpose, physical, social, intellectual and spiritual) are incorporated into the services we provide and embraced.
Responsibilities
- Assist in recruiting, hiring and training of the Assistant Program and Outreach Director.
- Participate in Personalized Service Plans (PSP) and refer to the Resident Profile and addendum for every new resident.
- Ensure a variety of appropriate and personalized activities that incorporate all dimensions of wellness, available throughout the day and evening with resident care associate participation.
- Maintain the PSL Activity & Program Guidebook, tracking daily and noting which residents exhibit interest and participate in which activities.
- Use the resident’s demographic profile information to organize small groups or clubs that address similar needs and preferences.
- Recognize changes in resident condition and take appropriate action.
- Assist residents to and from activity rooms or life skills and other normalizing life‑enriching activities and routine engagement.
- Maintain an inventory of activity and programming supplies, games, programs and crafts.
- Schedule, communicate, assist with facilitation and record monthly resident council.
Requirements
- Associate degree in Business Administration, Human Resources Administration, Gerontology or a related field (preferred).
- 1–3 years’ experience in senior living or skilled nursing activity & event planning, including moderate to large event planning responsibilities outside the senior living industry.
- Demonstrated leadership capabilities.
- Knowledge and experience in Assisted Living industry and dementia care (preferred).
Physical Requirements
- Stand/walk an average of 4–6 hours per day.
- Sit an average of 2–4 hours per day.
- Drive 30–60 minutes, 1–2 times a week.
- Lift up to 50 lbs. frequently and up to 150 lbs. occasionally; carry up to 40 lbs. frequently and up to 70 lbs. occasionally, with a height of 3–4 feet and a distance of 30 yards.
- Use hands for repetitive simple grasping, pushing, pulling and fine manipulation.
- Ability to bend, squat, kneel, climb frequently and reach occasionally up to 3 feet.
