Essential Responsibilities

  • Deliver supportive services tailored to individuals with mental health symptoms.
  • Assist clients in creating actionable plans to address recovery obstacles.
  • Complete accurate and timely documentation.
  • Participate in trainings and team meetings as required.
  • Collaborate with community partners, caregivers, physicians, and other providers to coordinate care.
  • Work with clients and care teams to develop crisis prevention plans and person-centered treatment goals.
  • Adhere to privacy regulations protecting health information.
  • Maintain professional communication with clients, team members, and external agencies.
Education and Experience
  • High school diploma or equivalent required.
  • Must be at least 18 years old and at least three years older than the youth served.
  • Self-identification as a current or former primary consumer of mental health services is essential.
  • Minimum of one year in stable recovery.
Preferred Qualifications
  • Two years of experience working with adults in a health-related field.
  • Certification as a Peer Support Specialist is a plus.
Licenses and Certifications
  • Completion of state-required training.
  • Valid driver's license, insurance, and reliable vehicle.
Skills and Abilities
  • Strong organizational and interpersonal skills.
  • Intermediate proficiency with MS Office.
  • Ability to mentor and support others.
  • Effective oral communication and problem-solving capabilities.

Physical Requirements
Light work involving occasional lifting up to 25 lbs and frequent lifting up to 10 lbs. Standing or walking may be required 11-25% of the workday.
