Physical Therapist (PT) - Home Health

We are seeking an experienced Physical Therapist for a Home Health assignment in Ladson, SC. The PT will manage a caseload within an assigned territory, providing in-home care to improve patient mobility, strength, and functional independence. Responsibilities: 1. Provide direct patient care per treatment plans 2. Document care and progress in EMR 3. Coordinate with interdisciplinary teams and families 4. Supervise PTA and home health aides Requirements: 1. Active South Carolina PT License 2. AHA BLS Certification 3. Minimum 1 year of PT experience 4. Home Health experience required 5. Valid driver’s license
