Registered Nurse Case Manager

We are currently hiring Registered Nurse (RN) Case Managers for full-time opportunities in Fort Walton Beach, FL and Tallahassee, FL. If you have a strong acute care background and are passionate about coordinating patient care and improving clinical outcomes, we'd love to speak with you.

Position Details
Locations: Fort Walton Beach, FL & Tallahassee, FL
Schedule: Full-Time, Day Shift (8:00 AM 4:30 PM)
Fort Walton Beach, FL

Requirements:
Rotating weekends required.
Current, unrestricted Florida RN license.
Minimum of 2 years of recent Acute Care experience within the last 3 years.
Recent experience in one or more of the following:
ICU
CCU
Medical-Surgical (Med-Surg)
Long-Term Acute Care (LTAC)
Previous Case Management or Care Coordination experience is preferred.
Tallahassee, FL

Requirements:
Current, unrestricted Florida RN license.
Minimum of 3 years of recent experience in one or more of the following:
Case Management
Medical-Surgical (Med/Surg)
Telemetry (Tele)
Neurology (Neuro)
Intensive Care Unit (ICU)
Progressive Care Unit (PCU)
Emergency Department (ED)
OR
Minimum of 5 years of experience in:
Home Health (HH)
Insurance Case Management

Key Responsibilities:
Conduct comprehensive patient assessments and develop individualized care plans.
Coordinate care with physicians, specialists, therapists, and community resources.
Facilitate discharge planning and care transitions.
Educate patients and families regarding treatment plans, medications, and available resources.
Monitor patient progress and modify care plans as needed.
Maintain accurate and timely documentation in the electronic medical record (EMR).
Collaborate with interdisciplinary healthcare teams to promote quality patient outcomes and efficient utilization of healthcare resources.
Qualifications:
Current, unrestricted Florida Registered Nurse (RN) license.
Excellent clinical assessment, communication, organizational, and critical thinking skills.
Experience with care coordination and discharge planning is highly preferred.
Proficiency with EMR systems.
Valid Florida driver's license and reliable transportation, if required.
