HCA Solutions is looking for inpatient and/or outpatient Pharmacist to provide services at the VA Medical Center in Salt Lake City. This facility is located at The George E. Wahlen Medical Center is located at 500 S Foothill Blvd, Salt Lake City, UT 84148! This is a full-time W-2 position offering long-term placement, health benefits, and paid time off. Mon-Fri daylight with some occasional shift coverage based on facility needs. Role description Pharmacist functions consist of processing and dispensing of discharge and emergency room prescriptions. Areas of emphasis include: Drug Distribution Prescription verification and distribution. Unit dose medication preparation, verification, and dispensing. Controlled substance dispensing. USP 797 compliance for sterile compounding and IV room proficiency (IV admixture, TPN, chemo, cytotoxic, or other compounding needs). Discharge counseling and patient counseling, adapting information to the patient’s level of understanding. Pharmaceutical Care Services. Monitoring of drug therapy. Providing drug information. Medication reconciliation skills. Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve desired outcomes through direct interaction with patients and providers in assigned areas. Orders, performs, reviews, and analyzes laboratory tests and diagnostic studies needed to monitor and support patient drug therapy. Follows up with patients regarding lab or test results, discussing therapy plans, changes in medication, or referrals to primary care or specialty providers. Adjusts medication regimens based on pharmacokinetic and pharmacodynamic data. Follows up with patients on lab or test results to discuss therapy plans, medication adjustments, monitoring, and additional testing requirements. Monitors and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Actively participates in the VAADERS Adverse Drug Reaction (ADR) reporting system. Be a US Citizen. Is a Pharmacy graduate from an American Council on Pharmaceutical Education (ACPE) approved college or university pharmacy degree program. Have a current and unrestricted Pharmacist and Pharmacist Controlled Substance license in good standing in a state, territory, commonwealth of the United States, or the District of Columbia. Have a National Provider Identification (NPI) number. Have a NABP e-Profile number. Be proficient in speaking, writing, and understanding English. Be proficient in communicating with patients and staff in a professional manner, including customer service skills, phone skills, and medical record documentation skills. Be proficient in typing, keyboarding, and using office equipment. Display a cooperative attitude and effectively work in a team dynamic to implement the mission and goals of the VA organization. Have the physical ability to lift, carry, and stock pharmacy supplies. This includes transport of supplies to wards, cache, or individual prescriptions to the dispensing window. Prescriptions may range from individual bags to cases of product or supplies up to 50 lbs. Must be able to stand, walk, climb stairs, and procure product from high or low shelving areas. Must be able to perform these duties while standing for their entire shift and maintain safe body mechanics, especially while moving heavy items. Proficiency in drug distribution and control, pharmaceutical care services, and pharmacy practice management.
