ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on delivering affordable VIP care to seniors, aiming to transform healthcare for the neediest population. The ideal candidate will be outcomes-focused, entrepreneurial-minded, and dedicated to building trusting relationships with patients to achieve better health outcomes.

Practice Info

  • Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in a panel of up to 450 patients.
  • PCP acts as a health coach, helping patients set short and long-term health goals.
  • Regularly measured metrics include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures.
  • PCP leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office patients.
  • For patients unable to come to the office (in hospital, SNF, LTC, or homebound), PCP will engage with the transitional care team and others including case managers and acute/transitional-care physicians.
  • PCP will have an active role in the management of their center and will help cover for other providers.

Responsibilities

  • Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient.
  • Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care.
  • Engage with the hospitalist whenever one of their patients is in the hospital.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • Lead the care team for patients able to come to the office.
  • Engage with the transitional care team and other resources for patients unable to come to the office.
  • Take an active role in recruiting patients for the center and additional providers for the company.
  • Perform other duties as assigned and modified at manager’s discretion.

Compensation

  • Salary: $221,141 - $315,915
  • This position may also be eligible for bonuses or commissions.
