ChenMed is seeking an Associate Center Clinical Director to supervise and train primary care providers (PCPs) while also providing direct patient care. This role is crucial for maximizing core model execution, improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. The incumbent will play a key role in leading their assigned center's clinical outcomes and fostering a strong clinical-operations synergy and culture.

  • Care team consists of a care promoter (medical assistant), care facilitator, and care coordinator.
  • Engages with transitional care team and other resources for homebound patients or those in hospital, SNF, or LTC.

Responsibilities

  • Independently provide care for older adult patients with acute and chronic illnesses.
  • Take full accountability for patient care and outcomes, seeking specialist consultation when needed.
  • Engage with hospitalists when patients are hospitalized.
  • Perform assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
  • Lead care teams for patients able to come to the office.
  • Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly.
  • Actively manage the center and help cover for other providers.
  • Manage, mentor, and coach PCPs to deliver outstanding clinical outcomes.

Compensation

  • $238,832 - $341,189 Salary
  • This position may also be eligible for a bonuses or commissions.

Shift & Schedule

  • Flexible to work evening, weekends and/or holidays as needed.

Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
  • Current, active MD licensure in State of employment required.
  • A minimum of 1-year clinical experience in geriatric, adult or family practice setting preferred.
  • Board Eligibility is required.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
  • Once Board certified, PCP will maintain board certification in their terminal specialty.
  • Must have a current DEA number for schedule II-V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.
  • Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes.
  • Fluency in English, verbal and written.
