Compensation

- Minimum Income (Annual Estimated W-2 Salary or 1099 Payment): $525,000
- Maximum Income (Annual Estimated W-2 Salary or 1099 Payment): $575,000
- Sign on Bonus offered
- Amount of Sign-on Bonus: $30,000
- Sign-on Bonus commitment: Two yrs
- Malpractice Paid: Yes
- Amount of Malpractice Coverage: 1 mil/3 mil
- Employment Status: W-2 (Employee) or 1099 (Independent Contractor)

- Monday-Friday
