Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Current Physical Therapy license within the state of practice
• Arizona Fingerprint Clearance Card required at time of hire and must be maintained for the duration of employment
