Physical Therapist - Travel Contract

Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- CA license required
- BCLS required
- HCHB required
- 1 year of recent Skilled Home Health experience within the last 3 years required
- OASIS experience required
- Own transportation required
Assignment Details
- Monday through Friday, 8:00 AM to 5:00 PM
- Geriatric caseload
- May visit up to 6 patients per day
- 30 productivity points standard
- Occasional travel radius of 50+ miles
- 1:1 patient ratio
