RN MDS Nurse
A Healthcare Facility in the Syracuse, NY area is seeking an experienced MDS Coordinator / MDS Assessor to join our clinical team.
Responsibilities:
Complete accurate assessments, MDS documentation, and care plans in accordance with regulations and facility standards
Initiate and update individualized care plans as assigned
Create and distribute monthly care plan schedules in a timely manner
Maintain and update clinical assessments and documentation as required
Monitor and audit clinical records to ensure accuracy, compliance, and timeliness
Communicate documentation concerns and compliance issues to nursing leadership
Maintain strict confidentiality of patient and organizational information
Qualifications:
Prior MDS 3.0 experience required
Active New York State RN License required
Previous experience in a clinical care coordination or assessment role preferred
Strong organizational and time management skills
Solid computer and documentation skills
Excellent communication and interpersonal abilities
Ability to work collaboratively within a multidisciplinary team
