Job Title: Medical Coder (Physician Practice)

Job Summary
We are seeking a detail-oriented Medical Coder to join our Family Practice physician office. In this role, you will review clinical documentation from patient encounters and translate patient visits, diagnoses, and procedures into precise medical codes. Your expertise will ensure accurate billing, compliance with healthcare regulations, and optimal reimbursement for our practice.

Key Responsibilities
* Review physician notes for accuracy and completeness and assign accurate procedure and diagnosis codes for provider visits, labs, and in-office procedures.
* Apply current E/M guidelines, modifiers, NCCI edits, and payer rules to ensure compliance.
* Collaborate with physician and nurses to clarify ambiguous documentation and provide feedback on coding trends.
* Review and correct denied claims related to coding errors or medical necessity issues.
* Research payer policies (LCD/NCD), coverage rules, and denials; assist with appeals.
* Review invoices for accuracy and payer specific rules application to ensure clean claim submission.
* Maintain productivity and accuracy; document work according to office standards.
* Stay current on coding updates and regulatory changes (CMS, OIG, 21st Century Cures Act).
* Protect PHI and follow HIPAA privacy and security policies.

Qualifications & Skills
* Active credential from AAPC or AHIMA preferred. Coding training essential.
* Medical coding experience, specifically within a physician office or outpatient clinic setting preferred. Experience navigating payer websites would be helpful.
* Strong understanding of outpatient billing, ICD-10-CM, CPT/HCPCS, E/M 2021+ guidelines, modifiers, and NCCI edits. Familiarity with laboratory testing usage and with prescription medication helpful.
* Proficiency with Electronic Health Record (EHR) systems, Excel, and Word.
* High level of accuracy, strong communication skills for interacting with providers and co-workers, and problem-solving abilities. Able to learn and recall quickly. Adaptability to change. Pleasant demeanor and the ability to get along with various personalities.
