Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Michigan physical therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Home health experience preferred, including skilled home visits and documentation.
Experience using EPIC electronic medical record preferred.
Ability to perform home assessments, develop care plans, and work independently in patient homes.

Preferred Qualifications

Prior travel or per-diem home health experience preferred.
Experience with holiday rotation schedules preferred.

Assignment Details

Shift Hours: 0800-1700.
Work Days: Monday through Friday.
Holiday rotation applies; no weekends or holiday coverage expected per client note.
Electronic charting with EPIC.
