Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Michigan physical therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Home health experience preferred, including skilled home visits and documentation.
  • Experience using EPIC electronic medical record preferred.
  • Ability to perform home assessments, develop care plans, and work independently in patient homes.


Preferred Qualifications

  • Prior travel or per-diem home health experience preferred.
  • Experience with holiday rotation schedules preferred.


Assignment Details

  • Shift Hours: 0800-1700.
  • Work Days: Monday through Friday.
  • Holiday rotation applies; no weekends or holiday coverage expected per client note.
  • Electronic charting with EPIC.
