Physical Therapist - Travel Contract

This role involves evaluating patient function, developing and implementing treatment plans, collaborating with interdisciplinary teams, and providing patient and caregiver education in the home setting. You will document care using electronic health records and coordinate care consistent with agency policies and regulatory standards.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Massachusetts Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Minimum 1 year of certified home health experience within a certified home health agency.
• Minimum 1 year of case management experience.
• Ability to travel to cases as assigned.

Preferred Qualifications

• Experience with electronic documentation systems used in home health settings.
• Prior experience working with interdisciplinary home care teams.

Assignment Details

• Shift Hours: 0800-1630.
• Weekend rotations occur once every 6 weeks.
• Ability to travel to patient homes across assigned service areas.
