Onsite Contract THERAPY

Looking for a change of scenery and a chance to help others. Provide in-home physical therapy services to patients across the lifespan. Plan, implement, and evaluate individualized treatment plans to restore or maintain patient function and well-being. Collaborate with interdisciplinary team members and communicate with physicians, patients, and families. Utilize durable medical equipment and adaptive devices to meet patient needs and safety within the home environment. Document Plans of Care, progress notes, phone orders, and other clinical records in a timely manner. Follow infection control and universal precautions. Travel locally as needed and perform physical tasks such as lifting, transferring, standing, and walking as required by patient care.


Minimum Requirements

  • Current State Physical Therapist license required to start the assignment.
  • Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an APTA‑approved program required.
  • Proof of current CPR (BLS) certification required.
  • Current driver’s license, reliable transportation, and vehicle insurance required.
  • 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
  • Ability to perform physical demands of home health visits including lifting, transferring, bending, and prolonged standing/walking.
  • Must read, write, and speak fluent English.
  • Client requirement: ability to travel locally and work flexible schedules including home visits.
  • Client requirement: competence in documenting Plans of Care, progress notes, and coordinating with physicians and interdisciplinary teams.


Preferred Qualifications

  • Previous home health experience preferred.
  • Experience with neuromuscular, neurological, and orthopedic diagnoses preferred.


Assignment Details

  • Position provides home health physical therapy visits with local travel required.
